Make Import Addresses Work Properly

describes a method but it doesn't work. The screens grabs are not up-to-date for a start, but at the end having gone through it perfectly, my address book is still empty.

I have been through the process of uploading our recipient address database (quite why this has to be done as an order, which then needs deleting eludes me).
The result is an address book with all the fields OTHER THAN EMAIL AND PHONE NUMBERS, meaning our mail recipients can't receive notifications.
These fields are definitely in the CSV and, I assume, crucial to the process, so can someone enlighten me, please?
